How Dental Laser Therapy Works

Laser-tissue interaction is selective: different wavelengths are absorbed differently by water, pigment and mineralised tissue. The clinician chooses the laser type, power and pulse settings for the intended tissue and procedure. Diode and carbon-dioxide lasers are commonly associated with soft-tissue treatment, while erbium lasers can also be used for selected hard-tissue procedures.

A photothermal effect can cut tissue and coagulate small blood vessels, creating a clearer operating field. A photochemical effect can reduce bacterial load, while low-level laser therapy, also called photobiomodulation, uses lower energy to influence cellular activity, inflammation and healing rather than remove tissue.

The effects depend on wavelength, dose and clinical technique. Laser treatment must be planned and delivered by a trained clinician with appropriate eye protection and safety controls.

Clinical Uses in Modern Dentistry

In periodontology, lasers may be used alongside scaling and root planing to reduce bacteria in periodontal pockets and manage inflamed soft tissue. They can also support gingivectomy and gum reshaping. Laser therapy is an adjunct to thorough periodontal care, not a substitute for removing plaque and calculus or maintaining effective daily oral hygiene.

Soft-tissue applications include frenectomy, exposure of an impacted tooth, removal or reshaping of selected tissue and uncovering a dental implant. Coagulation may reduce bleeding, and some procedures may require fewer sutures. Suitability depends on the lesion, location and need for laboratory examination; suspicious oral lesions require proper diagnosis rather than treatment based on appearance alone.

In endodontics, laser energy may supplement conventional irrigation to reduce microorganisms in complex root-canal anatomy. In restorative dentistry, suitable hard-tissue lasers may remove selected decay or prepare a cavity with less vibration. Conventional instruments are still required for many restorations and root-canal procedures.

Comfort, Healing and Patient Experience

Precise energy delivery can limit unnecessary disturbance of nearby tissue. Many patients experience less vibration than with a conventional drill, and coagulation can reduce bleeding during soft-tissue treatment. Some procedures may need less local anaesthetic, but anaesthetic is still appropriate whenever required for comfort.

Photobiomodulation may reduce inflammation and post-operative pain and support soft-tissue healing by influencing cellular activity and local circulation. Clinical reviews report benefits in selected indications, but response varies and faster or pain-free healing cannot be guaranteed.

Lasers are not suitable for every dental procedure. Large restorations, many forms of tooth preparation and numerous surgical steps still require conventional instruments. Equipment cost, clinician training and the need to select the correct wavelength also limit when laser treatment is appropriate.

Possible effects include temporary soreness, swelling, sensitivity, burns or delayed healing if energy is not appropriately controlled. Treatment close to teeth, bone, restorations or sensitive structures requires careful settings and technique.
